Good Faith Estimate Notice
The No Surprises Act ( OMB control Number:0938-1401)
The No Surprises Act protects you from unexpected medical bills for certain services. This law
ensures that individuals who are uninsured or self-paying for services receive a Good Faith Estimate
(GFE) of the expected cost of their care before receiving services.
Your Rights Under This Law:
- You have the right to receive a Good Faith Estimate explaining how much your medical or mental
health care will cost before you receive services.
- Healthcare providers must give you this estimate at least one business day before your scheduled
service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the
charges.
- You have the right to ask for an updated estimate if your treatment plan changes.
Who This Applies To:
- Individuals who do not have insurance
- Individuals who choose to self-pay for services
- Individuals receiving out-of-network care
How to Request a Good Faith Estimate:
If you are uninsured or choosing to pay out-of-pocket, you can request a Good Faith Estimate from
your provider before scheduling your appointment.
